How to Enroll your Child in School
Student Enrollment Process
Out-of-district transfers:
If your child is enrolling in a Clark County school for the first time (has never attended a public school in Clark County, Nevada), you need the following items to complete your child's registration:
1. Proof of address:
A proof of address is required. A recent utility bill (gas, power, or water) that has your SERVICE ADDRESS listed, a lease agreement, or a sales contract are considered acceptable forms of proof.
Please be advised that a telephone bill, cable bill, credit card bill, or driver's license are NOT acceptable forms of proof.
(If you are living with a friend or relative, please contact the Registrar's Office for a Residential Affidavit form.)
(People who are homeless or living in shelters may obtain enrollment information by calling the districts' Title I Homeless Program at 799-8547.)
2. Proof of child's identity:
An original birth certificate, a passport, or a certified birth card (issued by Clark County Health District).COPIES will NOT be accepted.
3. Up-to-date medical information:
Up-to-date medical records indicating that your child has had, or at least started, the following series of immunizations:
- Four DPT shots (but one more if last dose was given before 4th birthday)
- Three polio doses (but one more if last dose was given before 4th birthday)
- Two MMR shots (one dose must be given on or after the 1st birthday)
- A child enrolling in seventh grade is required to have the Bordetella Pertussis vaccine ("Tdap"). The child must receive at least one dose of a Bordetella Pertussis-containing vaccine after age 10.
For student new to Nevada schools:
- Two Hepatitis A shots (2nd dose must be given at least 6 months after the 1st dose).
- Three Hepatitis B shots (must have a minimum of 4 months between 1st and 3rd dose).
- One chicken pox (Varicella) shot (must be given at 12 months of age or older; 2 doses required one month apart for children receiving first dose on or after age 13 years). Physician verification required if student has had the chicken pox disease.
4. Student withdrawal form:
The transfer document that states the student's grades at the time of withdrawal.
Providing the following information will help ensure that your child is enrolled promptly and placed in the appropriate classes:
- Any test data or a recent report card that shows the name and address of your child's last school.
- Previous records regarding placement in a special program and/or class.
In-district transfers:
If your child is transferring from one Clark County school to another (within our district), you will need to provide the transfer papers from the school of withdrawal together with proof of residential addressto complete your child's registration.
Transportation is provided to all students who live more than two (2) miles from their zoned school. For more information regarding transportation for your child, please call 799-8111, the Transportation Department.
Withdrawing your child from school . . .
- Parents must fill out a withdrawal request form located in the Registration Office.
- Parent’s photo identification is required to withdraw a student from school.
- The student must remain in school (1) one entire day to go through the withdrawal requirements.
- Classroom books must be turned in to all teachers.
- Library books must be turned in to the library before withdrawal can be completed.
- The completed pupil release and transfer form will be given to the student to take home at the end of the school day. Parents will need to take the necessary documents to enroll the student in another CCSD school.
